At OCWMG, our goal is to make the disability application and process as smooth as possible. We assist you with all areas of your disability leave from start to finish.

California State Disability Insurance (SDI) is a partial wage-replacement insurance plan for California workers. The SDI program is state-mandated and funded through employee payroll deductions. SDI provides affordable, short-term benefits to eligible workers. Workers covered by SDI are covered by two benefits: Disability Insurance (DI) and Paid Family Leave (PFL).

File a Disability Insurance (DI) Claim OnlineThe state is now recommending that all claims be processed online.

  1. The Disability Insurance (DI) program provides affordable, short-term benefits to eligible workers who suffer a loss of wages when they are unable to work due to a non-work-related illness or injury, or due to pregnancy or childbirth.

    For Pregnancy related claims: the state allows you to take off work 4 weeks prior to your due date, and will pay you 6 weeks post-partum for vaginal delivery and 8 weeks for C-section delivery.

    For Gynecologic related claims: depending on the procedure that was performed, the physician will notify you with the anticipated time of medical leave prior to your surgery.

  2. SDI Online provides customers with electronic and automated options that are simple to use and available 24 hours a day, 7 days a week.SDI Online is convenient and secure. The new system reduces claim processing time, provides immediate electronic confirmation of forms submitted, decreases costs in paper and postage, and includes security safeguards to detect and manage fraud and abuse.
    To  initiate your claim for disability a one-time registration is required. Visit:

    **Upon completion of your DI/EDD form online, please provide the Disability Coordinator with your receipt number.*

    EDD will not all you to submit your online claim any earlier than the first day of your disability. To check the status of your claim you can log into your EDD online account or call the EDD office directly at (800) 480-3287

  3. To file a DI claim by mail, you must complete and submit a Claim for Disability Insurance Benefits form, DE 2501. To order a claim form online, visit Online Forms and Publications or call 1-800-480-3287 or 1-866-658-8846 (En español). We also have state forms available if needed.

    To avoid delays in claims processing, please complete the form as follows:

    • Use black ink only.
    • Type or write clearly within the boxes provided.
    • Fill out only the claimants portion of the form (Part A).
    • Do not fax the form.
    • Please bring your form to the office for our Disability Coordinator to complete the Physician’s portion. We will take care of mailing the completed form. If you have submitted your claim online, it is not necessary to complete a paper application.
    • Do not mail this form to the EDD if you have already submitted your claim online.
  4. Paid Family Leave (PFL) was established for workers who suffer a loss of wages when they need to take time off from work to care for a seriously ill child, spouse, parent, registered domestic partner, or to bond with a new child. Beginning July 1, 2014, California workers may be eligible to receive PFL benefits when taking take time off work to care for a seriously ill parent-in-law, grandparent, grandchild, or sibling.

  5. SDI Online provides customers with electronic and automated options that are simple to use and available 24 hours a day, 7 days a week.SDI Online is convenient and secure. The new system reduces claim processing time, provides immediate electronic confirmation of forms submitted, decreases costs in paper and postage, and includes security safeguards to detect and manage fraud and abuse.

    *Please note that our office does not need to fill out any portion of your FMLA/Baby Bonding Claim. *PFL cannot be applied for prior to your maternity disability ending.*

  6. To file a PFL claim by mail, you must complete and submit a Claim for Paid Family Leave Benefits form, DE 2501F. To order a claim form online, visit Online Forms and Publications or call 1-877-238-4373 or 1-866-658-8846 (En Español).

  7. Aetna, Sedgwick, Matrix, FMLA Works, etc.
    Please provide your Disability Coordinator with your forms.

    Short Term Disability and FMLA Forms (Employer Forms) will be handled as requested. Please make sure you complete and sign your portion. If you would like your forms to be faxed directly to the recipient; please make sure to include the fax number with your forms. In order to process your forms in a timely manner, please include the followng information on a seperate sheet of paper:

    1. First and Last name (if not already on the forms)
    2. First day of your Disability or Leave
    3. Date of Birth
    4. Provider
    5. Last 4 digits of your Social Security Number

    ***The turnaround time for completion of all forms is 7-14 day. Please submit your forms in advance if possible to ensure that they are completed by the deadline provided. You can submit your forms by fax to 949-581-9158, Attn: Disability or Email: ocw.disability@hoag.org. (Form(s) will be completed by the order it was received.)

    There is $15.00 processing fee for each form that has been completed (exp. EDD, FMLA,work forms, independent ins. company, etc.) Payment will be collected at the time your forms are submitted. Copies will be sent upon request.
